    Southern Rivers is proudly registered as an NDIS provider, and we offer a range of services to people living with disability. We can support you in your daily life, help you access everything that your community has to offer and open up opportunities to build your skills and reach your goals. Plus, we have a wide network of partners we collaborate with to offer allied health and other therapies. With Southern Rivers, you have a wide team of people supporting you and your family to make the most of life and reach your full potential.

  • What is included in an individualised support plan?

    An individualised support plan details your unique needs and goals and illustrates how funded supports can improve your quality of life. This plan may include assistance with daily living tasks, skill development, community engagement, home modifications and more.

  • How is my NDIS funding determined?

    Your NDIS funding is based on your individual needs and goals, which are assessed during your planning meeting with a Local Area Coordinator (LAC) or an Early Childhood Early Intervention (ECEI) Coordinator. We help you prepare for these meetings and can accompany you for additional support.

  • Can I change my NDIS plan if my needs change?

    Yes! The NDIS allows for flexibility in case your needs or goals evolve over time. You can request a review of your plan at any stage, ensuring you receive the most suitable support possible.
